Alcohol withdrawal in Tunisia: a detox never attempted alone.
Every substance has its own withdrawal, and alcohol’s is one of the few that can become dangerous: stopped abruptly, without medical supervision, it exposes the patient to seizures and to delirium tremens. So stopping is never decided in advance and never alone — it is prepared, and it is supervised. The signs of withdrawal
- Tremor, sweating, anxiety, nausea and insomnia, from 6 to 12 hours after the last drink.
- Needing a drink in the morning for the day to start, or attempts to stop alone that have already failed, several times.
Two complications make this withdrawal serious — and justify the hospital:
- Seizures possible within 48 hours of stopping.
- Delirium tremens between 48 and 96 hours: confusion, hallucinations, fever.

The care at the clinic
Withdrawal is inpatient: benzodiazepines on tapering doses under monitoring, hydration, vitamin B1 to prevent neurological complications. The withdrawal syndrome is treated, not endured. The stay follows the same path for everyone:
- Admission assessment: clinical examination; blood, liver and heart work-up; screening for associated disorders — anxiety, depression, sleep, suicide risk.
- Medically supervised withdrawal: tapering doses, vital signs checked several times a day, prevention of seizures and delirium.
- Stabilisation: sleep falls back into place, mood lifts or reveals what it was hiding — this is where the disorder masked by alcohol is treated.
- Work on craving, then the aftermath: motivational interviewing, cognitive behavioural therapy, a written plan for the tipping moments, and close follow-up consultations after discharge.

How long?
The acute phase lasts 5 to 7 days; fatigue and disturbed sleep persist for several weeks. A stay of two to four weeks additionally allows sleep to stabilise, the underlying disorder to be treated and the work on craving to begin; the duration is reviewed every week with the patient.
These durations are indicative: they vary with how long the drinking has gone on, the quantities, age and general health. The protocol is decided after examination.

It is often a relative who calls first, and you do not need the patient’s consent to ask for advice: a first call serves to describe the situation and gauge how urgent it is.
